Land Application Site Reporting Form - This form is used to report a land application site to IEMA. This form is completed once – not annually. It identifies the field to be used and reports the baseline radium concentration of the field. Instructions are on the last page. A commercial applicator may fill out and submit this form on behalf of the wastewater or drinking water treatment facility.
Landowner Acknowledgment Form - This form is used to notify landowners that the biosolids they are receiving contain radium. Once signed by the landowner or their designee, it is returned to IEMA. Land applicators may find that some landowners desire additional information and explanation regarding the nature and origin of radium in sludges. IEMA is willing to assist upon request. Community Water Supply Media Reporting Form - This form is used by community water supplies that do not land apply. The form is submitted annually and details the radium concentration, volume, and disposition of any backwash sludges, lagoons, or spent media that was disposed of during the previous calendar year.
